Community Living and Support
If you are living with a disability and require 24-hour accommodation and support, we can support you to live in a suburban house within the local community.
Is this program for me?
We can work with you if:
- you are aged from 18 to 65 and are living with a disability
- you have a referral from the Department of Human Services.
How can the program assist me?
Before you move into a house there will be a transition meeting, where we will explain to you:
- how our service works
- what we must provide you in the house
- your rights and responsibilities.
We will then work with you to:
- develop an individual plan that outlines your goals, needs and dreams, and what Melbourne Citymission can do to support you to reach them. Your plan might include: how you would like to spend your leisure time; the interests that you would like to follow; the skills that you would like to learn.
- provide you with options so that you can make informed decisions about your life and future.
Please Note:
You may like to get a family member, a friend or someone that you trust to help you when we meet with you to develop your plan.
