Acquired Brain Injury Community Access Program

If you are living with an Acquired Brain Injury (ABI), we can support you to get involved in your community and get the most out of local support services.

Is this program for me?

We can work with you if:

  • you are aged from 18 to 64 and have had an ABI for more than two years
  • you are on a low income or a Disability Support Pension and have no compensation as a result of your brain injury
  • you are living in Melbourne’s north or east.
How can the program assist me?

The Community Access Program is made up of two services, including:

  • case management
  • outreach support.

Case Management
Our case managers will work with you (and those important in your life) to:

  • maintain or increase your independence
  • give you information about services and options that may help you to reach your goals
  • access formal and informal services and supports.

We can also speak on your behalf when needed and we can work with you so you can feel confident to speak for yourself.

Please note:
To use this service you need to complete a formal application and submit it to the Information and Referral Case Manager.

When you first contact us you may be placed on a waiting list until one of our workers become available. You will be informed if there will be a waiting period.

We will meet with you regularly, develop a plan and review your plan regularly to ensure it meets your needs.

We do not provide funds (money), but if required, we can apply for funding that may assist you to meet your goals.

We can support you until you reach your goals, or until we agree you no longer need the support of your Case Manager.

ABI Outreach Support

Our Outreach Worker can work with you in your own home to support you to increase your independence and meet your goals relating to accessing the community. This could include:

  • learning how to use the train or bus by yourself
  • learning how to cook or how to manage your money
  • joining and attending a local class or group.

Please note

You can only work towards achieving one outreach goal at a time.

If you want to use this service you must have a Case Manager.

Your Case Manager should help you to identify your outreach goal and fill in the referral form.
