Acquired Brain Injury Maintaining Access Program

If you are living with an Acquired Brain Injury (ABI) and have previously had case management, we can assist you to make sure that things keep running smoothly and help maintain your independence.

Is this program for me?

We can work with you if:

  • you are aged from 18 to 64 and have had an ABI for more than two years
  • you are on a low income or Disability Support Pension and have no compensation as a result of your brain injury
  • you live in Metropolitan Melbourne
  • you have previously received support from a case manager.

How does the program work?

The Maintaining Access Program (MAP) case manager can work with you (and those important in your life) to assist with any problems or issues that might come up. The MAP case manager aims to make sure that the services and supports that are already in place continue to run smoothly.

The MAP case manager can assist to:

  • make sure that the services and supports in your life don’t break down
  • organise new services if you need them
  • live your life as independently as possible.

Please note:

We can provide this support on a long-term basis.

To use this service, you need to contact the Information and Referral Case Manager.

When you first contact us, there may not be a worker available. There is no waiting list for this service however, you can ask us to contact you when a worker becomes available.

We do not provide funds (money), however we can apply for funding that may help you to meet your goals if required.

We can support you until you reach your goals, or until we agree you no longer need the support of your Case Manager.

Acquired Brain Injury

We work alongside people living with an Acquired Brain Injury, providing tailored support to assist them to reach their goals.

We provide case management, outreach and secondary consultation for children, young people and adults using a family sensitive and person centred approach.
