Melbourne Case Management Services (MCMS)
If you have received compensation as a result of your Acquired Brain Injury (ABI), or are able to self-fund your services, then we can support you to participate in your community, maximising your quality of life.
Is this program for me?
We can work with you if:
- you have compensation as a result of a motor vehicle accident or workplace injury; you received a payout/compensation; or are able to purchase services independently
- you live in metropolitan Melbourne or Greater Geelong.
We work with children, young people and adults, aged from 5 to 65.
How can the program assist me?
MCMS can provide case management, assessment reports, and co-ordination of rehabilitation programs.
A case manager will work with you (and those important in your life) to maintain or increase your independence, and access formal/informal supports as required.
We will:
- discuss your goals with you and draw up a plan to meet these goals
- give you information about the services and options that may help you reach your goals
- speak on your behalf when needed and assist you to increase your skills to speak for yourself
- meet with you regularly to make sure that your plan is meeting your needs. If needed your plan can be updated and changed.
Please note:
MCMS is fee-for-service, that is, the service is not free and arrangements regarding payment will need to be made. We can liaise directly with your funder or financial intermediary regarding payment if this is appropriate.
You need to complete a formal application to get support from this service.
We can support you until you reach your goals, or until we agree that you no longer need the support of your case manager.
When you first contact us, if there is no capacity for your referral to be accepted, alternate fee-for-service case management options will be provided.

Acquired Brain Injury
We work alongside people living with an Acquired Brain Injury, providing tailored support to assist them to reach their goals.
We provide case management, outreach and secondary consultation for children, young people and adults using a family sensitive and person centred approach.
