Reconnect
Reconnect is an early intervention program that aims to prevent youth homelessness.
Who does the program support?
The program works with young people:
- aged from 12 to 18
- living, attending school or have connection to the local government areas of Maribyrnong, Brimbank and Moonee Valley
- at risk of homelessness or newly homeless.
How does the program work?
Reconnect provides a range of services that includes:
- support for young people to remain connected to family, school, training and their community
- case management
- assistance and advocacy support, which may include issues arising from: family conflict, family mediation, education, income support, accommodation, employment and conflict resolution
- referrals for young people to other services such as: health care providers, Centrelink, legal and other community-based services within their local communities and alternative options / programs for young people within their local communities.

Early Intervention Homelessness Programs
The Early Intervention portfolio (homelessness) provides a diverse range of programs that responds to the needs of young people at risk of or experiencing homelessness. The intention of all programs is to provide a framework of practice that remains youth focused whilst maintaining an inclusive approach to a young person’s connection to family, significant other and their community. All programs provide direct work whilst undertaking community development initiatives.
