Disability Planning and Co-ordination Service

Utilising a case management approach, the Disability Planning and Co-ordination Service assists people with a disability and others who are important in their life, to develop a plan with goals and strategies. It provides information about services and supports, and supports individuals in accessing services and supports, that cover areas in their life that they feel are important.

Is this program for me?

We can work with you if:

  • you are aged from 0 to 64 and living with a disability as defined by the Disability Act 2006.
How can the program assist me?

We can work with you, and others who are important in your life, to:

  • discuss your goals
  • put together a plan with goals and strategies that cover areas in your life that you feel are important
  • give you information about services and options that may be useful in reaching the goals you have said are important to you
  • assist you to access these services and supports for as long as they are needed
  • speak on your behalf when needed and help you to speak for yourself
  • go over your plan regularly to make sure that it is meeting your needs. We will update and change you plan if you think it is not meeting your needs.

Please note
A limited amount of funding may be available to assist people financially disadvantaged as a result of their disability.

Our service is time limited and will end when your goals have been met or you no longer require support.

If you are seeking support from this service you need to make a referral to Melbourne Citymission’s Case Manager – Intake. Intake is open Tuesday, Thursday and Friday. You may have to be placed on a waiting list until a worker becomes available.

When a Case Manager becomes available they will work with you to develop goals and assist with finding a range of services and supports to meet your needs.
