The Outreach Service
Melbourne Citymission’s Outreach service assists people with a disability to become more independent in their community. Outreach is flexible and support is based on individual needs.
If you are living with a disability, we can help you to develop the life-skills you want so you can be more independent in your community.
Is this program for me?
We can work with you if you are:
- living with a disability
- aged from 6 to 64
- living without paid support
- live in the northwest region of Melbourne
How can the program assist me?
The support we provide you is based on your needs.
An Outreach Worker can work with you on specific tasks. We can:
- help you develop the life-skills you want
- help you to develop day-to-day skills like: budgeting, using public transport, using a diary, house cleaning and cooking.
We will visit you in your home to:
- work out the things you can do
- talk to you about the things that you would like to learn
- find out what things make it hard for you to reach your goals.
After this first meeting we will then draw up a plan of the things you want to achieve with your outreach worker and how we will seek to achieve them together.
