Disability Support for Acquired Brain Injury (ABI)

  • Statewide Acquired Brain Injury Case Management Service
  • Melbourne Citymission’s Statewide Aquired Brain Injury (ABI) Case Management Service supports people with an ABI for a period of time after brain injury. Case managers in this service support people in accessing services, maximising their quality of life so they can participate in the community as fully as possible.

  • Acquired Brain Injury Community Access Program
  • The Acquired Brain Injury (ABI) Community Access Program, supports people aged 16 -64 following an acquired brain injury living in the Northern and Eastern regions of Melbourne.

    People are supported in accessing services, and developing skills to maximise their quality of life so they can participate in the community as fully as possible. The Community Access Program is made up of three services, Case Management, Outreach Support and the Maintaining Access Program

  • Statewide Acquired Brain Injury Paediatric (SAPC) Co-ordinators
  • The Statewide ABI Paediatric Coordinators assist people working and living with children and young people with Acquired Brain Injury (ABI). The services offered include; provision of information, training, mentoring, support, resources and community liaison/development.

  • Neighbour-ring
  • The Melbourne Citymission Neighbour-ring program supports adults with an Aquired Brain Injury (ABI) through local group social interaction. The group meets every two weeks, with the activities chosen by the members. A Community Development Worker and a Peer Development Worker facilitate the group and engage with local services and events.

    If you are living with an Acquired Brain Injury (ABI), our Neighbour-ring program can support you to make more friends and be part of a wider group in your local neighbourhood.

  • Acquired Brain Injury Maintaining Access Program
  • If you are living with an Acquired Brain Injury (ABI) and have previously had case management, we can assist you to make sure that things keep running smoothly and help maintain your independence.

  • Melbourne Case Management Services (MCMS)
  • If you have received compensation as a result of your Acquired Brain Injury (ABI), or are able to self-fund your services, then we can support you to participate in your community, maximising your quality of life.

Acquired Brain Injury

We work alongside people living with an Acquired Brain Injury, providing tailored support to assist them to reach their goals.

We provide case management, outreach and secondary consultation for children, young people and adults using a family sensitive and person centred approach.
