Statewide Acquired Brain Injury Case Management Service
Melbourne Citymission’s Statewide Aquired Brain Injury (ABI) Case Management Service supports people with an ABI for a period of time after brain injury. Case managers in this service support people in accessing services, maximising their quality of life so they can participate in the community as fully as possible.
If you are living with an acquired brain injury (ABI), we can support you to participate in the community, maximising your quality of life.
Is this program for me?
We can work with you if:
- you are aged from 18 to 64 with a moderate to severe brain injury and are about to leave hospital/rehabilitation
- you have no compensation as a result of your brain injury
- you are aged from 5 to 64 and are referred for case management through the Slow to Recover Program.
How can the program assist me?
We will:
- discuss your goals and draw up a plan to meet these goals
- give you information about the services and options that may help you reach your goals
- speak on your behalf when needed and assist you to increase your skills to speak for yourself
- meet with you regularly to make sure that your plan is meeting your needs. If needed your plan can be updated and changed.
Please Note
We do not provide funding (money), but we can apply for funding for supports that may assist you meet your goals.
Our service is time limited and will end when your goals have been met or you no longer require support.
You need to complete a formal application to get support from this service.
When you contact us, you may have to be placed on a waiting list until one of our workers become available.

We work alongside people living with an Acquired Brain Injury, providing tailored support to assist them to reach their goals.
We provide case management, outreach and secondary consultation for children, young people and adults using a family sensitive and person centred approach.
