  • Creating Connections
  • Creating Connections supports young people aged 15 to 25 who are homeless or at risk of homelessness in the NorthWest region. The program has a strong focus on early intervention.

  • Finding Solutions
  • Finding Solutions is an adolescent mediation and diversion service that works with young people and their families to identify and resolve behaviours and issues that are placing their relationship at risk of breakdown.

  • Young and Pregnant Parenting Program
  • Young and Pregnant Parenting Program (YaPP) provides supported playgroup and case management for young parents (from 15 to 25 years of age) and their children who are at risk of or experiencing homelessness.

  • Family Reconciliation Mediation Program
  • Family Reconciliation Mediation Program (FRMP) is a Statewide community-development and brokerage program that assists young people who are at risk of homelessness or are experiencing homelessness to improve their relationship to family and significant others.

  • Gateway Reconnect
  • Gateway Reconnect is a statewide early intervention program that aims to prevent youth homelessness. Gateway Reconnect is based at Frontyard Youth Services.

  • Western Reconnect
  • Western Reconnect is an early intervention program that aims to prevent youth homelessness.

Early Intervention Homelessness Programs

The Early Intervention portfolio (homelessness) provides a diverse range of programs that responds to the needs of young people at risk of or experiencing homelessness. The intention of all programs is to provide a framework of practice that remains youth focused whilst maintaining an inclusive approach to a young person’s connection to family, significant other and their community. All programs provide direct work whilst undertaking community development initiatives.
