Creating Connections supports young people aged 15 to 25 who are homeless or at risk of homelessness in the NorthWest region. The program has a strong focus on early intervention.

Who does the program support?

Creating Connections supports young people aged 15 to 25 who are homeless or at risk of homelessness.

How does the program work?

Creating Connections works in two main areas:

  • Housing Homelessness and Life Skills Component
  • Youth Focused Housing Placement Workers.

Housing Homelessness and Life Skills Component

The role of the Housing Homelessness and Life Skills Component is to support the development of an integrated service response in partnership with youth specific homelessness service providers and the Local Area Service Networks.

This includes:

  • ensuring access to a range of interventions that are easily accessible to service providers working with young people aged 15 to 25
  • providing a wrap around response/package
  • assisting/promoting more sustainable outcomes.

This work also includes the co-ordination and delivery of living skills programs designed to maximise the success of young people in taking up sustainable independent living. The team is currently in a consultative phase designed to contribute to the development of these living skills programs.

Youth Focused Housing Placement Workers

The role of the Youth Focused Housing Placement Workers is to provide short-term intensive responses to homeless young people at a range of service entry points to determine with them the next step to access housing and/or support.

Housing Placement Workers provide direct service delivery to young people who are accessing the homelessness service system. The service work with the integrated service responses that are being developed as well as any future access points/initiatives that may be established in the region.

Early Intervention Homelessness Programs

The Early Intervention portfolio (homelessness) provides a diverse range of programs that responds to the needs of young people at risk of or experiencing homelessness. The intention of all programs is to provide a framework of practice that remains youth focused whilst maintaining an inclusive approach to a young person’s connection to family, significant other and their community. All programs provide direct work whilst undertaking community development initiatives.
