Post Care Support Statewide Information and Referral Line
If you are 16 to 21 years old and are leaving state care, or if you have left state care, you can call 1300 leavingcare (1300 532 846) for information and support with accessing: accommodation; centrelink; identification; health; legal and much more.
To be eligible to gain entry into Victoria’s Post-care support Program, young people MUST:
- be between the ages of 16-21
- be currently on, or were on, on a Custody to Secretary Order or a Guardianship Order with the Department of Human Services (DoHS) on their 16th birthday (or there after)
- require assistance in their transition to independence.
The program aims to provide information and referrals to young people and or other professionals re: eligibility and nearest post care support service.

Homelessness Accommodation and Support Services provide a range of support to young people, adults and families who are at risk of homelessness or are homelessness.
The aim of Homelessness Accommodation and Support services is to empower service users to work towards independence. We respect and value the decisions of our clients so we can provide the information, advocacy and community links that best meet their needs.
