Disability Case Management Service
This service provides intensive case management, respite and support to children and adults with severe disabilities and their families. Individual workers assist families to plan, acknowledging the unique situation and special needs of each case. By combining several disability programs into one, the Disability Case Management Service can provide a family-focused pathway of care for people with disabilities aged 0 to 65 in every local government area in the Northern Metropolitan region.
