Our case management services

Everybody’s situation and story is unique. That is why each young person who walks through Frontyard’s door is welcomed and receives personalised and holistic support.

Frontyard has three case management teams tailored to each young person’s unique situation. These case management teams are, the Intensive Case Management Team, Young Women, Families, and their Children’s Team and Frontyard’s internal Circuit Breaker Case Management Team.

No needs are too complex.

Our three housing case management teams, work to identify pathways out of the homelessness system for young people aged 16 to 24. The teams provide time and space to engage with young people and then work with them to help them develop a care plan to enable them to achieve their goals.

Our case managers are passionate and utilise innovative ways to provide support and to ensure that a holistic care plan is developed and timely and intensive support is provided to assist the young person from crisis to stability and homelessness to a home. The team are experts in providing assistance and links in some of the following areas:

• health and wellbeing
• mental health and wellbeing
• community connection
• employment and education
• family reconciliation
• family violence
• parenting and maternal health
• shared, social and transitional housing
